Bridging the gap between health behavior change theory and clinical practice.

About Us

HealthChange Associates is a leading consulting and training organization and thought leader in the area of health behavior change, patient-centred care and self-management support - particularly in the area of chronic disease prevention and intervention.

Over the last 10 years HealthChange Associates has developed a practice model (HealthChange® Methodology) that enables clinicians and health services to embed client-centered communication and behavior change support into their clinical practice; education programs; primary and community care service; acute care and other service delivery in a time-efficient and clinically-effective manner.   

It has historically been a major supplier of training and program implementation assistance to chronic disease prevention, early intervention and management/self-management support programs run in most states in Australia.  It has worked with elements of state government health services, divisions of general practice, not-for-profit entities, community health services and private health organizations (including health insurers, diabetes organizations and corporate entities).  Examples include the NSW Get Healthy Information and Coaching Service, the WorkSafe Victoria WorkHealth Coach Program and the Sydney-based Live Life Well Diabetes Prevention Program.

Since the workshop started at Deakin University in 2006, over 7,500 clinicians in Australia and overseas (including Canada and Denmark) have attended the HealthChange® Core Training Part 1 (CT1) 2-day workshop.

HealthChange Associates has delivered services to all types of clinicians (including medical specialists, general practitioners, practice nurses, community and district nurses, dietitians, diabetes educators, exercise physiologists, physiotherapists, occupational therapists, social workers and psychologists), in many different types of industry sectors (primary care, community sector, acute care, corporate sector). Input to the development of methodology has been gained from the thousands of clinicians that have undertaken the training and dozens of health services with whom we have worked.

The HealthChange® Practitioner’s Guide is used as a textbook for postgraduate courses at Deakin University and the University of Western Australia.

HealthChange Associates Partners

HealthChange Associates works with partner organizations to deliver training in the HealthChange® Methodology of health behavior change to individual health practitioners and organizations globally and provides a range of consultancy and customization options to assist organizations to develop programs, or to integrate health behavior change support into existing clinical services, education programs and other health services.Consultancy options include assistance with program design, skills development support and tracking, quality assurance systems and audits, and program evaluation. We invite collaboration with organizations that would like HealthChange Associates to provide support for research projects.

Our Canadian Partners

Alberta Health Services Chronic Disease Management Service 
Alberta Health Services is piloting the HealthChange® Clinician Peer Trainer Program (train the trainer) for health care providers within their chronic disease management and primary care services to develop more behavior change support skills and self-management skills. 

Ottawa Regional Cancer Foundation

A key international partner is the Ottawa Regional Cancer Foundation who is transforming cancer care in Eastern Ontario with the opening of the country's first Cancer Survivorship Centre empowering survivors to be active participants in their health and well-being through and beyond cancer.  Services at the Maplesoft Centre are available without referral and without cost to those diagnosed with cancer, their families and caregivers. Through individual and group coaching, tailored programs and relevant workshops individuals come to discover their personal needs and concerns, gain new knowledge and understanding of the impacts of cancer, and identify and select strategies that are customized to their unique situation. The Cancer Foundation's Maplesoft Centre offers a safe and welcoming environment that encourages the exploration of options and where the survival of integrity, autonomy and dignity are always possible.
